Why Is Haze Worse for Elderly Parents in Malaysia?

    Haze is worse for elderly parents in Malaysia for two reasons, and neither is about being frail. Adults aged 65 and over are more likely to already live with a heart or lung condition, and, as the US Environmental Protection Agency puts it, "the body's ability to respond to health challenges generally declines with age." That is why Malaysia's Department of Environment and Singapore's Ministry of Health both name older adults specifically in their haze advisories.

    On 11 August 2026, 11 areas in Sarawak were recording unhealthy API readings, with Serian at 189, close to the very unhealthy band.     Why does haze affect an older body differently?

    Two things stack. The first is arithmetic. The US EPA states it plainly: "Adults ages 65 and older are at greater risk, because they are more likely to already have chronic heart or lung disease." Haze does not create the condition. It makes an existing one harder to live with.

    The second is the body's response. Fine particles travel deep into the lungs and the smallest pass into the bloodstream, so haze shows up in older people as tiredness or a racing heart, not only as a cough.

    At what API reading should my parents stay indoors?

    Malaysia's Department of Environment attaches health advice to each API band, and older adults are named in it. Singapore's Ministry of Health runs a stricter row for the elderly. Combined:

    API Status What the advisories say
    0 to 100 Good to moderate Normal activities. The window for errands.
    101 to 200 Unhealthy DOE: "limited outdoor activities for the high risk people". Singapore MOH: minimise prolonged or strenuous outdoor exertion.
    201 to 300 Very unhealthy DOE: "Old and high risk people are advised to stay indoor and reduce physical activities."
    Above 300 Hazardous DOE: "Old and high risk people are prohibited for outdoor activities."

    Read the second row again. At an API of 150 most adults feel nothing and carry on, while Singapore's advisory already names the elderly at that level, and Malaysia's DOE names them from 201. Note the API is not a PM2.5 concentration: DOE calculates a sub-index for each of six pollutants and publishes the highest one, which during haze is usually the particle sub-index.

    Should my elderly parent wear an N95 mask?

    Ask their doctor first. This is the one piece of haze advice that is genuinely different for older adults, and both regulators say so. The US EPA's 2026 wildfire smoke and ash factsheet is direct: "Wearing a respirator can make it harder to breathe. If you have heart or lung problems or are pregnant, ask your doctor before using a respirator." Singapore's Ministry of Health, in its haze advisory updated in 2026, says that elderly people and those "with severe lung or heart problems who have difficulty breathing at rest or on exertion should consult their doctor as to whether they should use the N95 mask."

    If the answer is yes, three details decide whether the mask does anything:

    • Two head straps, never ear loops. EPA is blunt: an ear-loop mask is "not designed to seal tightly to the face and will not protect your lungs from smoke."
    • Clean shaven where it touches. "A good seal is not possible with facial hair," which matters if your father has kept his moustache for 40 years.
    • Off at the first sign of trouble. Dizziness or breathlessness means cleaner air, no mask. A respirator also raises heat risk here, so short trips only.

    What actually helps at their house?

    More than the mask does, because most older Malaysians spend a haze episode at home. A closed room is not automatically a filtered room. EPA's "clean room" is one room, usually a bedroom, kept shut, kept free of particle-making activities such as cooking or smoking, and fitted with a portable air cleaner sized for that room and run on its highest setting. EPA lists older adults among the people a clean room may help most, alongside children and people with heart or breathing problems. Set it up in the room your parents sit in during the day, not the one they only sleep in.

    Why will they not tell you the haze is bothering them?

    Because "are you okay?" gets "okay je" every time. Older parents here routinely under-report how they feel, partly out of habit and partly because they do not want to burden a busy child.

    Ask narrower questions. Did you sleep through, or wake up coughing? Did you finish your usual walk, or turn back early? Malaysia's Ministry of Health lists the haze symptoms worth naming out loud: itchy throat and coughing, difficulty breathing, stinging watery eyes, runny nose with frequent sneezing, itchy skin, and chest pain.

    Two of those are not wait-and-see. The US CDC notes that for someone with a heart condition, "palpitations, shortness of breath, or unusual fatigue may indicate a serious problem" and is a reason to contact their doctor. Singapore's MOH puts it broadly: people who are not feeling well, especially the elderly and those with chronic heart or lung conditions, should seek medical advice. A cough that outlasts the haze matters too, which is why a cough lingers after the haze clears.

    What about their daily routine?

    Everything above is about exposure, and exposure decides how a haze week goes. A well fitted mask, a closed house and a purifier in the right room do more for your parents than anything they can eat or drink. Nothing replaces them.

    What a daily routine adds is consistency, which older parents are usually good at.     Frequently asked questions

    Can my parents still do their morning walk during haze?

    Up to an API of 100 there is no restriction. From 101 to 200 Singapore's MOH advises the elderly to minimise prolonged or strenuous outdoor exertion, so a shorter, slower walk at the cleanest hour is the compromise. From 201 upward Malaysia's DOE advises older and high risk people to stay indoors. Walking a corridor or a mall keeps the habit without the exposure.

    My parent has a heart or lung condition. What should be different?

    Three things. They act one API band earlier than everyone else in the house. They ask their doctor before using an N95 mask, per both EPA and Singapore MOH guidance. And they keep taking prescribed medication exactly as advised during a haze episode, which is the first line of Malaysia's Ministry of Health haze advisory for high risk groups.

    Does the air purifier need to run all day?

    In the room they use, yes, on the highest fan setting they can tolerate. EPA's clean room guidance is to run it continuously and to pick one that does not produce ozone. An hour before bed does very little.
