Why Does My Cough Get Worse at Night?
A cough gets worse at night for three ordinary reasons: lying flat lets mucus collect at the back of the throat, the airway is naturally twitchier in the small hours, and in Malaysia the bedroom is often the coldest and dustiest air a person breathes all day. Most night coughs are not a new illness.

Why does a cough get worse when you lie down?
Gravity stops helping. Upright, the mucus your nose and sinuses make all day is swallowed without you noticing. Flat, it pools at the back of the throat, and the throat clears it the only way it can. Stomach contents also travel upward more easily when you are horizontal. The airway itself narrows slightly in the early hours, part of a normal daily rhythm, so the same irritation produces more coughing at 3am than at 3pm.

Does a cold bedroom make a night cough worse?
There is evidence that it does, and it is the most Malaysian-relevant finding on this page. A 1996 study in the European Respiratory Journal recorded night cough in 100 British school-age children who had all been identified as wheezy. It tested that cough against a list of asthma measures, including bronchial reactivity and overnight oxygen saturation, and found no association with any of them. What it did find, in the authors' words, is that "it was associated with lower overnight air temperature."
Two honest limits: the cohort was children already known to wheeze, and an association in one study is not proof of cause. But the direction is worth acting on where the bedroom is the one room deliberately chilled every night. If the aircon sits at 18 degrees and the cough peaks between midnight and dawn, raising it two degrees for a week costs nothing.

Is it the aircon or the mattress?
Often both, since they arrive together: a closed, cooled bedroom recirculates its air through one filter for eight hours, and Malaysian humidity makes bedding a comfortable place for dust mites. See also how often to service your aircon.
Change one thing at a time: week one, raise the temperature; week two, hot-wash everything on the bed. Changing both at once tells you nothing.
Why is a night cough hard to judge?
Because what gets remembered and what actually happened are not the same. A 1993 study in Pediatric Pulmonology compared diaries against tape recordings over seven nights in 15 children with asthma who had reported troublesome night symptoms. Cough was reported on 66% of available nights and recorded on 90% of them.
The study offers no explanation for the gap, and its group was small and specific, so it is not a rule about all coughs. What it shows is that the record of a night cough and the experience of one differ. Track duration on a calendar rather than judging by how a bad night felt.
When does a night cough need a doctor?
On the same terms as any other cough, with no discount for happening at night. A 2014 study of 117 adults at one Malaysian primary care clinic records that physicians here are advised to identify the cause of a cough lasting more than 2 weeks. Sooner, if any of these, among the NHS urgent warning signs, is present: a cough worsening quickly, feeling very unwell, chest pain, difficulty breathing, or coughing up blood. A night cough in a child that comes with wheezing, or returns month after month, is worth a doctor's opinion rather than another week of thermostat experiments.
The habit families keep through a broken week of sleep
Sleep is what a night cough costs, which is why households want something steady rather than something for the bad night itself. Does sleeping propped up on pillows help a night cough?
Raising the head end of the bed helps more than stacking pillows, which bends the neck rather than tilting the body. Put something solid under the legs at the head of the bed. It is a comfort measure and does nothing about the cause.
