How Long Should a Cough Last? A Malaysian Guide

    How long should a cough last? Most coughs from an ordinary cold settle within two to three weeks. The threshold that matters in Malaysia is earlier than the one printed on most international health sites: doctors here are advised to find the cause of any cough lasting more than two weeks.

    How long does a normal cough last?

    Doctors sort coughs by duration, not by how bad they sound. According to a 2014 study of 117 adults at one Malaysian primary care clinic, published in the Journal of Family Medicine and Primary Care, "a generally accepted definition of chronic cough is an episode of cough lasting more than 8 weeks". Below that sit two shorter bands.

    The UK's National Health Service sets its non-urgent threshold at three weeks: among the NHS signs for seeing a GP are a cough lasting more than 3 weeks and unexplained weight loss.

    Why is the Malaysian threshold two weeks, not three?

    Because of what has to be ruled out here first. That same study records the local rule plainly: "In Malaysia, physicians are recommended to promptly identify the cause of a cough lasting more than 2 weeks, with pulmonary tuberculosis as a primary concern." The recommendation it cites is the Ministry of Health Malaysia's own tuberculosis guideline. The three-week rule is British and American; Malaysia sits in a different disease environment, so checking starts a week earlier.

    That study found the commonest cause among its patients was post-infectious cough at 35.9 percent, followed by cough related to a common blood-pressure medication at 12 percent. Most long coughs are the tail of something ordinary, or a side effect nobody connected to a tablet. Both need a doctor.

    How long does a child's cough last?

    Longer than most parents expect, which causes a lot of unnecessary worry. A 2013 systematic review in The BMJ reported that cough "had resolved in 50% of children at 10 days, and 90% by 25 days".

    Those are percentages of children, not stages of one illness. Day 10 is when half of children have stopped coughing; day 25 is when nine in ten have. The authors suggest reading them in absolute terms, as the time for half of children to get better and the time for nine in ten to get better. A child still coughing in week three is inside the range the evidence describes, and the warning signs below apply at any age.

    What signs mean see a doctor now, whatever the calendar says?

    Duration is the default. These override it. Among the signs the NHS lists as needing an urgent appointment are a cough that is very bad or worsening quickly, feeling very unwell, chest pain, swollen painful neck glands, difficulty breathing, and coughing up blood. Whatever you are handed at the counter, an antibiotic is usually not the answer: the NHS states plainly that "Antibiotics are not normally prescribed for coughs".

    Why does a haze-season cough feel like it never ends?

    Because the irritation keeps being renewed. The Ministry of Health Malaysia's haze guidance lists an itchy throat with coughing first among the six symptoms it names. An airway still settling after an infection, then meeting smoke particles at school pickup, restarts from behind. That is why a cough that would have gone quietly in April drags through September.

    How long is too long for a cough in Malaysia?

    More than two weeks is the point at which Malaysian primary-care guidance says a cause should be identified, earlier than the three-week threshold used in the United Kingdom, because the Malaysian primary-care literature names pulmonary tuberculosis as the primary concern here. Beyond eight weeks a cough is defined as chronic. Any warning sign, such as coughing up blood, chest pain or difficulty breathing, means see a doctor immediately whatever the calendar says.

    My child has coughed for three weeks. Is that normal?

    It is within the range the evidence describes. A 2013 systematic review in The BMJ found cough had resolved in 50% of children at 10 days and 90% by 25 days, so week three falls between the 10-day mark, when half of children have stopped, and the 25-day mark, when nine in ten have. See a doctor anyway if the child is breathless, feeding poorly, feverish for days, or getting worse rather than slowly better.

    Does yellow or green phlegm mean I need antibiotics?

    No. Colour is a far weaker signal than most people believe, and the NHS states that antibiotics are not normally prescribed for coughs. What decides treatment is the whole picture your doctor sees, not the shade in the tissue.

    Can a cough come back every haze season?

    Yes, and it commonly does. A recovering airway meets fresh smoke particles and the irritation restarts. The Ministry of Health Malaysia lists an itchy throat with coughing among the symptoms it expects during haze. Reducing your exposure indoors matters more than anything you can swallow.
